As a state-licensed childcare center, we are required to keep the following mandatory admission forms on file for all enrolled students. Please click the links below to download, print, complete, and return the forms to ABC Learning Center.

Completed forms may be:

• Emailed to our office

• Dropped off in person at ABC Learning Center

We appreciate your help in keeping our records current and in compliance with state regulations.

Admission Information (Form 2935)

This form is required for all children enrolled in a licensed Texas childcare center.

Immunization Record

A current immunization record must be on file prior to your child's first day.

Vision and Hearing Screening

All children turning 4, must have a vision and hearing screening on file with the center.

You can schedule this with you child’s doctor at a well-child checkup.
